Are you in-network with Horizon?

That depends on your specific plan, because Horizon sells many different plans through employers and directly to individuals. Call us with your card and we will tell you whether we are in-network for your plan, and what your share of the cost would be either way. Being out-of-network does not mean you cannot use your benefits with us; it usually means your share is higher.

Dental coverage is often separate from your Horizon medical plan, and the two can have different member IDs. Bring both cards if you have them and we will work out which one covers your visit.

What Horizon plans usually cover

Most PPO dental plans sort treatment into three levels, and your plan sets the percentages, the yearly deductible and the annual maximum. These are typical figures, not a promise of coverage; we confirm your actual numbers when we check your benefits.

  • Preventive care, such as exams, cleanings and routine X-rays: often covered at or near 100%.
  • Basic care, such as fillings and simple extractions: often covered at about 70–80%.
  • Major care, such as crowns, bridges and dentures: often covered at about 50%.

Horizon plans sold to small employers and individuals often include waiting periods for major work, and some have a lower annual maximum in the first year. We check both before we plan treatment.

How to check your Horizon benefits

  1. Have your card ready: member ID, the policyholder’s name and date of birth, and the group number if your plan comes through an employer.
  2. Call us at (201) 533-0005, or send the details through our appointment form.
  3. We check your plan and explain what it covers, what it does not, and what you would pay, before you commit to treatment.
  4. After your visit, we file the claim with Horizon for you.

You can also sign in to your Horizon member account to see your annual maximum and how much of it you have used this year. If your plan runs on the calendar year, unused benefits usually reset on January 1.
